I would list it on the form. If you're honest and say what you learned from the incident, I don't think it will hurt your application. If you hide it, and they find out, they won't accept you

Yea, I agree. It one of those things that they may not find out if you don't tell them about it. However, if they do find out after you haven't told them, then you are totally screwed...

Do all schools require a dean's certification before matriculation? Or do they call up your undergrad institutions themselves?
