So does it really matter where I go for my neuro residency if I do not want to go into academics and am currently not sure if I'll do a fellowship?

When deciding on where to go for med school, I kept hearing it was important to go to a highly ranked school. I have learned it probably would not have mattered where I went to med school.

Will it really matter as far as future clinicial opportunites and fellowships if I decide to go to UCI or some other "mid tier" program in a more desirable location vs say a ucla, stanford or u of wash.

It's a game of supply-n-demand. You will find that the higher you climb on this ladder, the market becomes more favorable to you. You will have choices of fellowship at places that you didn't get for med school.
If you go private practice, you can put yourself in the shoes of the partners and answer your own questions. What characteristics in the candidate are important to the practice? I would say accuracy and effiency are key. Not name of school. If you have solid letters from your PD, saying that you are good in clinical neurology and get your work done efficiently, you will have no problem getting a job of your choice.
Go to a residency where you like the call schedule, didactics, location, atmosphere, etc. You won't regret it.

Check out how well the faculty relates to the residents. Ask the residents if they would have selected that residency again if they had a choice. 10 nurturing faculty members are better than 30 %^&% faculty members. Your decision should feel ok with your gut feeling.
