I much prefer to extubate deep than not. If there is a reason not to... then you shouldn't. That simple IMHO.

I wasn't disagreeing (I prefer deep extubations as well), just wanted to make a point. Its important that residents understand that a T&A does not equal deep extubation. Without getting into details, had a resident some time ago pull a tube deep (not a T&A) which unfortunately resulted in a very bad outcome.

I just wanted to reemphasize to the new rookie residents who read this forum that deep extubations are EXTREMELY dangerous unless done 100% correct. I'm no expert myself (and def no where near the experience of an attending like sevo) but it takes precision, timing, and skill. NEVER experiment with the technique without an attending present.

That's jacked up man. We're there for ALL the extubations. If a resident pulled that Shiite and took out the tube deep on a kid w/o me in the room I'd annihilate him.
