deferring matriculation

Assuming you are considering deferring matriculation for a year, if admitted, is it wise to let MD/PhD committees know at an interview or could that negatively affect your chances of being accepted in the 1st place?

I wouldn't let them know until after you put down a letter of intent... Otherwise, they can just have you reapply again at no loss to them...
 
unless you think you are deferring for a scholarship like a fulbright/rhodes/marshall/gates/churchill etc... which will undoubtedly impress them - i wouldn't mention it until after you are accepted. this is my personal opinion, and i have no idea what the official line is.
