You do have to list all schools attended.
You should list your publication because research productivity is highly valued by at least some adcoms, particularly if your role at the time was as a student in a science program.
I think that in terms of why you left the PhD program, "In late 2017, I became very ill with seizures due to chemical exposure from a lab accident and rather than change labs, as was suggested to me, I chose to leave school and was hired to work as a [job title] in an underserved area. This reignited by interest in medicine as a career and led me to ...." So, that's what you've got for your personal statement and I think that's all you need to say about it.
As Lizzy pointed out, it’s all about the spin. I did a bunch of painful committee busy work ran medical QI for the hospital when I was in the .mil. It was uncompensated work nobody wanted to do. You bet your ass that I spun the F out of that garbage for 5 ⭐ fellowship and faculty jobs.
Thanks destriero for the reply and congrats on the career success! I will definitely be mulling over my PS QUITE a bit after I take the MCAT in a week.
Most important takeaway is to make lemonade out of the lemons in hand.
I was planning on contacting the PhD institution to give them the heads up that I was applying to med school, since I will have to list a contact for the research hours (a years worth, so approx 2000). Do you think this is a bit overboard or a good idea?
