I failed one of my classes when I took it pass/fail and now have to figure out where to put it on my application. I was suffering major personal issues that semester that resulted me in failing that class and getting a C in another. The reason I failed the course is that I had to take 5 classes per semester to maintain a 5-digit scholarship, so I switched the class to P/F instead of withdrawing from it. There were no make-ups allowed for the homework and exams. I turned in some of the homework and did not take the exams. I ultimately lost the scholarship because I failed the course...

My school's letter writing committee offered to explain this in my committee letter. Should I ask them to? Or should I talk about this in my application/secondary application?
The committee LOR could be a two edged sword. IF you have recovered from these deficits and have a strong, rising GPA trend, then this will help. Otherwise, it will simply draw attention to negatives.

You do NOT explain this in your PS; that's not the place the place for it, and no matter how they're written, they only come off as excuses.

Save explanations for prompts that specifically ask about how you dealt with adversity or specifically ask about poor grades.
