Congrats on having a positive vision of yourself and future career! In my view, D is the best choice while keeping you in cycle. You are likely will need to add non-MSTP MD/PhD programs in the middle of the Heartland. Your stats are lower than the averages for MD/PhD students.
I do not know much about your personal goals for E or G. State of residency plays an important role for MD-only schools... I hope that it works out for you...
