And your aren't applying for MD-PHD programs?
I have been debating it heavily. For how much time I work in my labs, countless grad students + my PIs say I would fit well within a program. I have the capabilities of most grad students even now, but I honestly don't believe right now that the Ph.D. is the "right fit" for me. Yes, I could get through a 4 year Ph.D. program, but my heart may not be in it. I may apply to MSTP for UMN Twin Cities and one/two more programs, but I know I can do research as an MD. Still have a lot of debating to do.

Your app pretty much screams "research" as it stands, but you can decide whether that is something you want to do. You do need to get back into community service. I would not worry so much about having your claim of hours validated as long as you make a good estimate. Just get more than 150 hours (minimum expectation). Also, the more you do something away from campus or independent of campus, the better.
