Can you provide more details and follow the template? It will help.

The details are important because ... I'll be frank first, cut down your reaches if you're coming in with a 512. It is a real stretch for Stanford, Case, Michigan, and I have to check the others.

Where in California did you grow up? Did any of the UC PRIME programs appeal to you? Why aren't you applying to CUSM (their average MCAT is 511)? Many UC schools favor applicants who grew up in their service areas.

How many of your 100 hours at your homeless shelter were done before starting college? For the WAMC, please separate them so we know how many hours were done after you started college. Ideally, you need 150 hours before applying to avoid getting screened out at most schools. More teaching will have minimal impact on your application.

Why do you feel medicine is your purpose?

Your reaches are unrealistic with a MCAT of 512. I suggest these schools with your stats:
Vermont
Quinnipiac
Albany
New York Medical College
Hackensack
Penn State
Drexel
Temple
Jefferson
George Washington
Virginia Commonwealth
Eastern Virginia
Wake Forest
Methodist (when it opens)
NOVA MD
Belmont
Tulane
Alice Walton
Roseman
TCU
Rosalind Franklin
Medical College Wisconsin
Western Michigan
Oakland Beaumont
Wayne State
California University
Kaiser
UC Davis
UC Irvine
UC Riverside (if you are from that region)
