Dentist LOR-everything i need to know?

Okay so I am not applying this cycle and not any time soon. (Probably in 2 years) I have shadowed a dentist for 60 hours and i was just wondering if i should ask him for LOR now or a couple months before i apply? Also, how do you go about asking a dentist? I know he knows its coming. He said something about writing me a letter. Also, I have heard there is a special form you need them to fill out along with the letter. Do you know anything about this or where i would find it. Any advice or help would be appreciated.
 
Ask now. Or atleast a year in advance. It's a pain to scramble and get them a few months before you apply. Use interfolio or something to store the letters.
 
Use interfolio. I didn't and I regret it. You can ask him when you are going to stop shadowing him. Interfolio will keep the recommendation on file for a while (2 years i think). When you are ready, instruct interfolio to send them out.
 
so does the dentist send the letter to interfolio and then they store it for a while and then i can send it out to d schools ????
also do you have to pay for interfolio
