Is it required for me to have a dentist rec on my app? I worked with a dentist last summer, but I'm not sure how 'fresh' I'll be in his memory after all these months.

Depending on the schools you're applying to, and if you go through a committee at your school for your letters, then you might not have to have a dentist letter. Out of my list, the only school I sent a dentist recommendation was Tufts, and only because they specifically emailed asking where it was.

For Tufts, would it suffice to have a note from a dentist documenting the number of hours shadowed or do they require a formal letter of recommendation?

Hmmm...So you want to be a dentist which means you have to go to a DENTAL SCHOOL...so yes, even if your target schools do not explicitly have a requirement for a LoR from a dentist, submit one. It would be foolish not to. Reacquaint yourself with the dentist with whom you shadowed. You still have a few months until the portal opens for 2011 applicants.
