I have a quick question.

Do dental schools ask you to submit something signed by your dentist, other than LOE, such as work experience verification form before matriculation?

My dentist is planning to move his practice soon, so I'm wondering...

it depends on the school...ohio state requires a separate form, but it was faxable, so location doesnt really matter

Yeah.. I know he could simply fax or mail it for me, but I know that he wouldn't do that.. For my LOE, I had to ask him in person several times before he finally sent it. That's why I'm concerned.
 
If you have applied to LLU, then you will need your dentist to sign a form they provide which confirms that you have volunteered/worked a certain number of hours with him/her.
