derm fellowships (particularly procedural derm fellowships)

I was just wondering: how competitive is it to get one of these fellowships, and what do programs that have these fellowships look at when deciding who to admit? Is it still all about step 1? Or is it about the prestige of the derm residenecy you come from? Or....is it about how good of a resident u were?
