I'm an incoming med student working on a case report with a doctor I met while working as a medical assistant. We're planning to submit it for publication, but I'm not sure how the fees usually works. Do students typically have to pay out of pocket, or is that something the doctor usually covers?

Note: from what I can tell, my school doesn’t offer any discounts or cover publication fees

Paying to publish is typical of a journal that allows for open access/creative commons over copyright (and many predatory journals also request pay to publish). But students should not pay to get their work published. The money should come from the PI's grants or institutional resources.
