diff btwn unpaid extracurrics vs. work experiences???

What's the difference between unpaid extracurriculars and unpaid work experiences? What would volunteer work at pharmacies count as? How about shadowing someone at a hospital? Getting confused as I fill out the app. Thanks in advance!
