Difference between full medical license and temporary permit?

I'm starting residency in June, and according to the residency program's many documents, have the choice between obtaining either a "Medical License" or "Temporary Training Permit." I won't be able to start residency without one of these.

What's the difference between the two, besides, I'm assuming, the price? Can I, as an incoming intern, even apply for a full medical license without passing Step 3?

Without at least a year of postgraduate training, you won't be eligible for full licensure.

From the website, Physicians FAQ:

What's the difference in the training required by American versus International programs? American and Canadian graduates are required to have one year of ACGME, AMA or RCPSC accredited post-graduate training. International Medical School graduates and Fifth Pathway applicants who graduate from medical school on or before July 1, 1985 must complete one (1) year of post-grad/residency training in the US in a program approved by the ACGME, AMA or the RCPSC. After July 1, 1985 international/fifth pathway graduates must complete three (3) years of training in an ACGME, AOA or the RCPSC of post-graduate training, because we do not have any way to effectively determine the quality of an applicant’s medical education from schools outside the United States or Canada. Both American and International graduates must pass all three steps of the USMLE examination.

I'm not sure why they give you the choice unless the form is written for all physicians coming to practice in the state.

Without Step 3 (and perhaps some additional state requirements; check Miklos' link above or http://www.fsmb.org) you are not eligible for a Medical license, but rather a temporary medical training (MT) license at this point in time.
 
Maybe it was a generic form for all physicians entering Georgia.

I guess I'll be going for the temporary medical training license then, since I haven't taken Step 3 yet. I thought this was to be completed during internship year, but I think at Emory, you have until the end of your PGY3 year to take it?!

Depends...we weren't allowed to advance on (ie, we could still work but 'on paper' we were still PGY2 and paid as such until we passed Step 3) to PGY3 until we had passed Step 3. Different state, program might have different rules. Lots of people do take it during their internship year, but there is no state law requiring it...since most of us tend to forget those things we aren't using day to day, if you aren't doing a medicine residency, you'd be better off taking it earlier rather than later.

The residency programs I interviewed with all cover the cost of getting the training license, which will cover you for all years of residency. If you want to moonlight, you may have to take Step 3 at the end of intern year so you can have your own license to cover you outside the training program.

My friend who is a chief OB resident didn't have to take Step 3 until this year. His program doesn't allow moonlighting, so there was no rush to get his own license. If you get your own license after intern year (but before completing residency), you usually have to pay the fees associated with getting your license, as your residency program will not cover it.
