Difference between IM and FM

What exactly is the difference between IM and Family medicine (besides the fact the IM does not deal with Peds/OB/surgery)? I know that an Internist can also be someones Family doctor, so what is the difference in terms of knowledge and the patients that they serve? Can someone be referred to an internist by a family doctor?
