I take responsibility for the medications I dispense, you prescribe them accurately and in accordance to evidence-based medicine. That's the deal.

When dealing in situations where the evidence is much more controversial and open to subjective discretion, then we can take "faith" into consideration.

Except legally, the pharmacist is responsible for the prescription as well. And sometimes dr's are not conscientious. I argued with a doctor once who had wrote for Fentanyl patch 50mcg for a 13 or 14 year old (I don't remember the exact age now) after having a procedure, I think it was a tonsillectomy. I call the doctor and suggest Tylenol/Codeine (this was years ago before Tyl/Codeine was contraindicated in kids after tonsillectomies.) Dr said the pt had already tried that and it wasn't working and they were in serious pain. I then suggested Percocet....Dr told me he "wasn't comfortable prescribing that." But he was comfortable prescribing fentanyl patches to a narcotic-naive 13 year old!!!! End result, he refused to change, I refused to fill. I gave the RX back to the mother and told her it would likely be fatal if given to her child. She was aghast and said she she would just continuing giving the Tyl/Codeine to the child.

So yeah. Just because you write it, doesn't mean it's going to get filled.

Full stop.

If we tell you something is a bad idea, you'd better believe we mean it and be able to articulate why you're willing to risk harming your patient.


Otherwise, if you give the bull**** "I've always done it that way!" Or "I'm the doctor™", I'll tell the patient we don't fill prescriptions from unqualified physicians that don't know enough to keep their patients safe and send them on their happy way to the next pharmacy
