Combined Dent/Med programs (there may be more.. these are the only ones I know of)
Case Western - 5-year program
Nova Southeastern - 6-7 year program
Sydney University - 7-year program (can complete either med or dent first and you apply for the second degree in your final year of the first degree. To be eligible you must graduate in the top 5-10% of your class from your first degree.)
Kings College London - 7-year program (Must apply to each program seperatly)
Other options
University of the Pacific - offers 5-year dental program to high school graduates and a 3 year dental program to those with 90 semester hours of college credit.
Columbia University - offers a 3 year dental program to medical graduates interested in either OMFS or Oral Med/Path.
McMaster University - offers a 3 year medical program for those with previous degrees.
VCU - offers a 2.5 year med degree program to anyone who has completed a dental degree and a 4-year OMFS residency at a non-MD optional program.
Why might you want both a dental and a medical degree?
You want to complete Maxillofacial surgery training - Current IAOMS (international OMFS) training guidlines reccomend obtaining both degrees.
