Do fellowship programs care about gap year between medical school and residency?

gonna need a little more information than that. it all depends on what you did that year and how you performed in residency afterwards.
 
Most people I've asked say no. Your performance in residency and any research you've done trumps basically everything from your medical school record except perhaps board scores since by the time you're applying that's 3 years back on your CV or more. That's what I've heard/read anyway. Someone with first hand experience could chime in. Gaps DURING residency are a different story though.
 
Unless you spent it in jail, no.

Thanks a lot for your input. One last question, when applying for fellowships through eras, how should I answer the following question "Medical Education/Training Extended or Interrupted?".

Tell them what you did during that gap year.

Regardless of what you say, they will probably assume the delay was probably because you didn't match the first time... but as long as it wasn't a year in prison, it won't matter.
