How difficult are the AOA versions of these to get into? Are most of these concentrated in the midwest?

It depends on the program. There seems to be a little more inbreeding when it comes to choosing their own residents first, but otherwise it's going to be about the same as any ACGME program.
and yes, outside of philly, most fellowships are in Ohio, michigan, and a few in Oklahoma.
Can you do a allopathic IM residency and then apply to the DO cario..etc spots? DOes this occur often?
It's possible, but I would imagine that it's not any easier than going from osteo residency to allo fellowships

What if you did your IM at a good allo spot like USC, Loyola, Rush, Albert Einstein..(other DO friendly spots)..etc would it actually be more difficult to match into a fellowship at the DO spot?
No, as already mentioned DrMom and I both know (the same person) who went to an allo residency and a osteopathic fellowship. It's not impossible, But you will run into the Rule 42 issue about getting your ACGME program AOA approved and all that hoop-la. If you search my started threads, You'll find that there are proposed rule changes which would make that extremely difficult should they pass.
I have no clue as I do not know anyone has tried this. But if you're good enough to get into one of those, why wouldn't you be able to get an allo fellowship?
I actually know a fellow that did his residency and cards fellowship at an allo program and missouri. He is currently an interventional fellow at St. James in suburban Chi.
Ultimatly you are gonig to be better suited to quit looking at it as what is the easiest way to get into a cards fellow, but to look at it by asking how much hard work is it going to take to get in any program period. Being an osteopath does not give you loop holes, it doesn't open the kingdom to easier fellowships to get into as they can be just as competitive. Buckle down and get to studying.
I anticipate I will be accepted to most DO schools (3.4 gpa and 30 mcat, plus masters degree).
He said he knew many other people that were not able to match into fellowships even though they were well qualified (grades, board scores, places they did residency) bc they were technically fmgs and nothing else.
I m still deciding wether to go St. Georges or DO school next year. I ve been accepted to St. Georges and I anticipate I will be accepted to most DO schools
