If I want to go in to competitive mid-tier residency, do I need to have publications?

youre going to have to be more specific. what specialty, where, etc.

Hm.. such as radiologist, dermatologist or Orthopedic. I have no clue about locations tho...

Radiology no, ortho yes, derm yes. Generally top programs yes regardless of specialty.
