I'm enrolled to a medical school in UK but English isn't my 1st language (and not my nationality), do I have to to TOEFLE english exams since English is practically my first language anyway.

Um, do you need TOEFL for what exactly?
If your question is "Do I need the TOEFL to apply to residency programs in North America", the answer is, and this is not intended to be unhelpful, it depends. As an IMG applying to a program in Nova Scotia, Canada, I had to get a letter from the Dean of Medicine stating that my program is taught entirely in English and so the TOEFL was waived (and the Dean's letter was priceless; his character was definitely there for those of us who know him. You could hear the tone of voice). Different provinces have different requirements, and I imagine so do different states. Some provinces require it of all IMG's regardless, others are willing to admit that if you studied in English, or came from Canada originally from an English speaking province, that odds are high that you speak English to a relatively high standard. Others are not.
If your question is "Do I need the TOEFL for something I didn't specify above which is not a residency application" then specify.
Cheers,

ugh...I'm sorry i was typing my thoughts out in a rush. Yeah, you're right what I meant was "do i need it for applying for residency"
