Does the DO letter of rec need to be from a DO whom you have shadowed or can it come from a DO who is a family friend? I know many schools allow character letters from someone who knows you very well (family friend, boss, etc.). Thanks.

I would suggest contacting the school(s) that want the DO letter of recommendation and ask for clarification. I personally would think that a LOR from a family friend who knows you well would be a better representation than someone you have shadowed once or twice. Perhaps you could shadow your family friend to cover all the bases.

When I did my LOR, I asked a friend of mine if I could shadow him a couple of times. His LOR was more as a "character reference" than about my experiences shadowing him.

I agree that the friend would be a better character reference. My family friend DO actually doesn't live close by so unfortunately I can't shadow him.
 
My DO LOR came from a DO that I had never met. He was a friend of my parents'. I sent him a copy of my resume, personal statement and an addressed and stamped envelope. Must have worked because I am gearing up to start my second year soon.
