Do Medical School Applications ask if you have taken online courses?

The transcript from that school might say "online" in the course description/classification, and you have to submit a transcript from every school where you took courses.
 
There was at least one secondary asking explicitly whether any of the classes I've taken have been online. Can't remember which school, but if you really care, I could check later.
 
I had 2 of 19 ask - but one of was only if the course was a pre-req. If it wasn't they didn't seem to want to know about it or care. Most don't ask, but I would still suggest only taking non-prereq classes online since they'll be able to see on the transcripts if you loaded up the 'hard' premed courses online.
 
But even my transcripts don't seem to indicate that a course was taken online. I wonder if it's just my university that doesn't show that.

at my school there were some science courses offered online but you would just watch recorded lectures online and submit written work online but all exams were taken in person and proctored. It seems that not all online courses are created equal. I would be wary of an online course that does not do the proctored exams thing

No my school also doesn't say if a course was online or in person on transcripts. So the only way a med school would know is if they specifically ask on a secondary
 
I had multiple secondaries ask whether my classes were online or in person, fwiw.
