depends on the school. some schools want to see the official transcripts after they accept you. they let pharmcas handle everything at first so they don't get thousands of transcripts at once, but once they accept you, they'll look at the transcripts of only the students that were accepted as a final check.

I had to send a transcript to my school after acceptance - at that point, they only had the PharmCas information. I had been done with undergrad for 9 years.
