When does this need to be done by? Do I have to shadow a DO and get the LOR before submitting the app?

They need to be in at the same time as your secondaries. If ur missing them they won't look at ur app until the letter is in if its a requirement for that school

Depends on the school. LECOM offered me an interview before I had the letter, but definitely wanted it submitted before the interview. Other schools absolutely require it before consideration of your app.
