I have 50 hours primary care and opthalomologist shadowing hours.

My stats 512/cgpa3.86/sgpa3.82. As a back up plan to apply to do schools. Is MD shadowing not enough for do application. I guess same recommendation letters hold good. what else I need to do differently/separately for do application.

You'll get in somewhere MD with those numbers, just don't apply to top-tier MD and you'll be fine.

Letters? Volunteering hours? Comm service hours? Leadership positions?

The well-rounded student is of the essence these days.

Letters from 2 research profs
1 from science Prof
1 from non science Prof
1 from EC activity coordinator
50 hours of shadowing
2000 hours of research
500 hours of clinical volunteering
400 hours of non clinical volunteering
700 hours in leadership position for the school organization
20 articles(poetry and prose) published in online journals.mostly health related.
Honors thesis publication on research project
As this is a gap year plan to take up a part time clinical job that will add another 1000 hours of paid employment
