Will I be able to match for programs without taking the USMLE?

This question comes up so much. The answer is, yes, you can match into ACGME programs without taking the USMLE. But, my disclaimer would be, if there are programs out there that interest you, check into them. Send the PD an email, ask current residents, etc. Some might want you to take the USMLE so just find out.
 
Just take the USMLE. You're studying for it by studying for COMLEX. Many PDs and committee members appreciate having the USMLE, especially since, from my experience, a lot of them don't know how to interpret the COMLEX.

Also, you never know what your specialty interest is going to be come ERAS time (personally, I switched last minute. A few of my friends did, as well). Let's say you decide you really liked that neurosurg or rads rotation in June/July/August of 3rd year, moreso than neurology. Your going to be glad you took step I of the USMLE.
 
If you expect to apply to competitive programs, take the USMLE.

Top programs will look for USMLEs when stratifying applicants and the DO status already puts you at somewhat of a disadvantage, which a good score will overcome.
