I trained (through ALACE) and volunteered as a labor doula at a birth center about 5 years ago. I probably did about 50 hours of volunteer work.

I don't have any documentation of this work...I could probably contact a couple of people who could say - yes, I remember that she did this (midwives, other doulas), but nothing official. Do I need anything official? I think this is pretty interesting volunteer work, and would like to include it in my CV. Thanks.

You won't need anything official. The school can always check on it if they are suspicious.

50 hours from 5 years ago will not be very helpful to you...what med schools look for is sustained and recent/current activity over brief and dated activity...just a word to the wise...

Oh, no, that's not my only volunteer work at all! I've done volunteer work since, and am currently volunteering weekly at a hospital. Actually, it was about 3 years ago, not that it makes much difference in terms of dates.

No one will ask for documentation. That being said, don't lie. Your reputation is a precious commodity. If anyone ever finds out you lied on your application, it is grounds for withdrawal from a program (even your degree if you graduated-- it has happened to people).

I would never lie! Just didn't know if I needed something "official" to even put it down.

on a related topic: i know that at least one SMP DOES check on EC's. It's a small class and the final applicants are checked.
