Does ERAS ask about remediation or failures not reported on transcript?

Does Eras have a question that asks the applicant to disclose history of academic remediation or unreported failures?
The transcript should not have any remarks concerning remediation, but I am wondering whether that is an empty gesture since programs will know about remediation anyway.

Huh, my school reports remediations and failures.

Some schools do not disclose remediations on transcripts but from what my recently matched friends have told me, ERAS requires you to disclose all failed classes regardless so either way it will come up.
