I think it would be interesting to work there. I want to actually practice though and I know NIH is more for research. But do they hire surgeons? I know the pay wouldn't be great but I think it would be interesting to work for them.
Your specialty is not particularly important as long as you have the qualifications they are looking for. Plenty of surgeons have a lab and do clinical work as well.

1) you can do research without a PhD
2) you can do clinical work with an MD/PhD
3) do not do an MD/PhD unless you have a strong desire to do research (in addition or instea of clinical work)
