No it should not at all at most programs. Most programs will rank you that day and no matter how many thank you letters, second looks you do, you won't move. The second look is for YOU, to see if you want to rank that program high. If you're torn between 2 or 3 programs then it is useful.

The second look is really for you. Programs don't care. Becareful, however, not to burden the program. I've seen programs get "irritated" when applicants demand or ask too much when coming back for the second look.
I think that the second look is actually specialty dependent. For most specialties, the second look is soley for you; but I do have a friend who is applying for plastics programs right now and he told me that a couple of PD's have told him that applicants will only be considered seriously if he comes for the second look.
