This upper level international public health class provides the option for a supplementary education trip to Nicaragua, where students can do health outreach and promotion in a community and for an NGO. You can get one additional credit hour if you choose to participate in this trip.

Would this count for an activity in your application? Would it be less ambiguous if I didn't receive the additional credit?

On the AMCAS application, there is a section called "experience". Going abroad as part of a class and for credit is as much an "experience" as doing research for which you receive credit.
