Does step 3 scores matter for IMGs who are trying to get H1b? If I pass Step 3 but drop by 50 points from my Step 1+2 will programs care?

Step 3 score is only important as a pass on first attempt. Even with a 76.
It does not matter much at all( maybe for fellowship if they match you side by side with a similar candidate, it makes more sense to some PD to pick the higher step 3 score. )
Otherwise just pass it.

What he said. Just pass the damn thing. For us (EM), we just had to "pass" to progress to second year, so you at least want to study enough to comfortably pass. I comfortably passed and scored fine but not as high as I did on 1&2. Nobody cares. I have no idea if it affects internal medicine sub-specialty applications. Someone might want to comment on that. If you don't plan on doing a fellowship or sub-specialty then I doubt it matters what you score, just don't "not study" because you need to pass it on first attempt.

I would ask on www.valuemd.com, a website by and for IMGs. This website mostly caters to US grads, I don't think we have a lot of knowledge about how the IMG match process, let alone the process of applying for a visa for residency.
