I will hopefully get a couple publications out of my research from M1-M2 years, but may not be first author on either. Will that matter when it comes to applying? I have a 226 step 1 and I saw that the average for last year was 230, how is that number determined and who calculated it if the step scores are supposed to be confidential? Is 226 good enough for a semi-competitive location?

it's an average. some are higher, some are lower. if you apply broadly, you'd be fine. being first author is nice in ophtho researh, but certainly not essential.
