I vote not research, though engineering stuff can be.
 
I did similar stuff in undergrad. I listed it under research. That being said, I also worked in research labs that were more basic-sciency. I'd say this would definitely add to your resume and be a good experience, but it would definitely be more "design"-y than researchy. I would count it as half doing research.
 
I think "hypothesis based" is what medical schools are looking for. If part of the project is research regarding the efficacy of this device in medical training and follows hypothesis driven data gathering and analysis to test its effect, such could be considered research.
 
Not research. You could list it under "other" as a team-based design project. I love hearing about these at interview, too.

Research is generally hypothesis driven. Usually basic science at the bench, sometimes chart review study or secondary data analysis of previously collected data (social science, medicine, etc), occasionally a field-based study in environmental science or whatever.
