answered my own question. for those of you who are wonderng also:

Letters of recommendation should be forwarded directly to the office of Admissions by the recommender and should include the form provided in the secondary application. Letters should not be submitted by the applicant.

Its official: it sucks to be ME.

FYI: My school collects individual letters and is very familiar with UK and we were told that we didn't need the form.

Call the school to clarify.
