Doing an DO internship and then another PGY1?

In order to practice in Pennsylvania I have to do a traditional rotating internship (D.O.) before starting my residency. I want to do Psychiatry which starts with its own PGY1. This seems like a conflict to me. Does anyone know how this works? Thanks...

If it is an ACGME Psychiatry residency you would have to repeat the PGY-1 year. You would also, I believe, have to apply for the Psychiatry program during your intern year. FYI- in general, residency programs do not like to take applicants with prior PG training because the government funding for their training slot is decreased.

Another option is to apply for the ACGME program directly and then petitioning for approval of your ACGME PGY-1 year under Resolution 42. Since it would save you the wasted rotating intern year, it is something you should consider.
 
Actually, for standalone internships (transitional years or traditional rotating internships) I don't think that your residency funding will be affected. My experience was that a lot of programs seem to respect the knowledge you've gained from an internship as long as you have a good explanation for why you did it.
