Hi everyone,
I ma dentist from india .In india we do not have seprate categorization in passport as middle name but if we writre name as s..... d....... g.... we take s.... as (first) d....(middle ) and g... as (last name).In the caapid appliaction detailing the personal information,only one letter ca be witeen in the middle name coulman and it is also mentioned as it is in passport. .But i have full middle name in passport so, should i write the first and middle name in first name column or first letter of middle name in middle colum.

i KNOW IT IS A SILLY QUESTION BUT DNT WAN 2 TAKE CHANCE IN FILLING THE FORM.

Write only initial.Suppose if your middle name is JOSEPH write only 'J'.
And don't mix your first name and middle name in a single box.
Last name = surname
Middle name=father's name OR husband's name
First name=Our name.I know some consider it as a silly ANSWER!!!

I'd say first letter of your middle name, or you can skip that field altogether as its not mandatory

can someone please clear this doubt- In the CAAPID application, in the ' letters of evaluation ' column, in the waivers section- do we have to mark the first option 'I waive my right to access' the attached letter of evaluation???
I have letters of evaluation from my professors sealed in envelopes and also signed across the seal. so, is that option in the application to be marked?? i hope im clear. please suggest whoever have already sent CAAPID or are doing it now. THankyou very much. Please hurry up...

They clearly mention that they will not accept anything else.
If your shadowing letter is one of your LORs then send it..or else, send the other stuff directly to schools.

According to the law, a student has the right to access his/her recommendation letter. So, its ur choice whether u want to waive that access or not. If you do not waive ur access to the letters you can always ask the university for a copy of the letter once submitted. But i have heard, it is always better to waive ur access to the recommendation letters....hope this helps...

do we need to send the supporting documents like notarized toefl and nbde score and other things which we mentioned in the application..i wud be thankful to u guys.

you can submit the CAAPID application online. mail them ece and LOR's along with the matching forms provided in the aplication. no need to send toefl and nbde score. they have to be sent to schools directly.

no, u need not send anything other than ece and LOR's. If any of your experience letter is LOR, then send it. rest all have to be submitted to schools.

I don't think we should write Dental camps as they are just a part of our Internship.All I have understood is mostly it depends on our scores I don't think adding this little things make a much difference.🙄

Include your dental camps experience in community service section. I did the same. Do mention that you conducted camps in your SOP.

I included my shadowing experience in practice of dentistry section as volunteer.
yess!! clip it or staple the matching form with the LOR.

I have send LORs with matching form by clipping them together hope if it works?
Some say write shadowing under 'Practice of dentistry' others say under 'other work'.I asked and CAAPID says choose your best option.LOL
