Hi, is there a way one can download the aacomas application to be sent in? I am getting severe eye strain trying to complete the process.

Not really that I know of.
Also, unless you have a typewriter or true wordprocessor that can type in a form, you would have trouble. You really want to type it out, something this important should look very professional.
If you have a monitor, a CRT, like a television, then your refresh rate may be too low, set it above 75 Hz and the strain may be less, above 85 Hz is good for most people. Maybe you could even use a computer with a LCD display as those dont have a refresh at all, you would feel better.
