I hope most of you interested in pharmacy, but not yet in it get Medscape.

Here is a link which reports the top 10 drug errors, which was a source of many discussions at the ASHP Meeting. The list is complied from many sources & resorted in many ways depending on what errors you're looking at - pt errors, pharmacist errors, administration errors, system errors & IMO - the most important factor - knowing how you, yourself can contribute to the error.

Hopefully, most others will read about this in more depth in the journal, but it gives you food for thought.

Thanks for the link. Drug errors are one of the main reason why it took me so long to decide if I wanted to apply to pharmacy school or not. Knowing that one slip-up on your part can kill someone or cause a life-long injury is very sobering.
