Seems pretty cut and dry from the inpatient side. He should have absolutely had DVT prophylaxis while inpatient. Can't speak for discharge though.

Acute severe ulcerative colitis is a huge risk factor for VTE, something like 5-6 times the baseline risk. Add in a major abdominal surgery, and you're a sitting duck.

Look like it was documented as ordered but not actually ordered. Definitely a systems issue. Very tragic.

Yup. See these people from time to time in Heme clinic especially post-resection(s) we gotta figure out which anticoagulant their remaining gut is fit to absorb.
