Early fills?

Hi all, If a regular customer was requesting early fills ( say,on day 26/27 for a 30 day supply previous month) for a C-2, would u fill it ,if it was covered by insurance?
To add another scenario, if shows up as an insurance reject and the customer wanted to pay cash, how would you respond?

but seriously, if they are three days early? it kind of harsh expecting someone to come on exactly day 30 to get it (ADHD med when kids have school and parents have work)


but anywho, just use professional judgement and be consistent

NYS makes this very clear. You can have a maximum 7 day buffer. So if one month you're 3 days early, and 4 days early the next, you're getting held to the day. The excuses about "oh but I have work, school, etc" aren't really holding water, because you have an extra 7 day supply at that point, and being held to the day means you still have plenty remaining.

I like that, not the law where I am, but it sounds like a reasonable guideline to follow.

This is for every new control prescription for the whole time they're on that med? So if they get a new rx for Adderall 10mg and get it 4 days early month 1 and then 3 days early the next month, they have to get it in on day 30 for the rest of the time they're on that med, even if it's years?

Theoretically, it's forever, yeah. In practice you go back a few months, and if they're early every month, you're going to hold them. If they're early one month, and then late the next month, no problems, because they reset their 7 days. It's just a good way to catch the chronic early folks, who are 3 days here, 3 days there, and by the end of the year basically fit in a whole extra Rx.

what's store policy? If store policy is 2 days and you always fill only 2 days ahead, then release it 2 days early.

there's no reason to release it earlier, people should know they're taking a controlled medication and it shouldn't be filled early w/o MD approval.

just think, you keep filling it earlier and earlier to the point where the patient truly starts abusing it and then it comes on to your shoulders. why was the pharmacist continuously filling the rx early?

Personally, i don't care if it's 3 days early. I don't care if i'm being an ******* to some degree. The fact is, they're there too early, and they need to wait.
