How difficult are the ECG's on the Step 1 exam ---> Is FA and BRS Physio sufficient to do well or are we expected to know the various complicated patterns associated with various cardiac diseases in each of the leads?

ECGs on Step I are a snap, very classic patterns that a monkey could recognize.

As for each lead, I doubt they would ask you specific disease states. However, they might give you a question with Einthoven's triangle, tell you that a cardiac vector is positive in Lead I, and negative in Lead II, and ask you to pick out the right diagram
