Definitely try to get some janitorial experience in there. Preferably shadow a custodian, but if there are only janitors in your area, you have to work with what your given.
In all seriousness, there are several categories you need to fulfill:
1. DO/MD Shadowing (preferably DO)
2. Clinical experience (something hands on. Could be hospital volunteering, but things like ER scribe or EMT are better)
3. Volunteering (you need both clinical and non-clinical volunteering. Hence, if you can be a volunteer EMT, you satisfy both clinical experience and clinical volunteering)
4. Leadership (preferably doing something healthcare related)
5. Research (not necessary to fulfill, but 1 year of research experience is pretty standard for a lot of applicants. Presentation at a national/international conference or publication for maximum gain from this EC)
6. "Other" (This is more to round out your application. It can include hobbies like playing music or traveling, as long as they can be substantiated with achievements. Or, it can include tutoring, teaching, or whatever else helps identify who "you" are)
