I believe this is a requirement for almost all US medical schools. The 90 hours is credit earned from an academic institution which, correct me if I'm wrong, means that AP doesn't count. It's a bit ambiguous because most schools don't explicitly say AP doesn't count, but I did find one school's website that said AP doesn't count towards the 90.
 
It is most likely 90 credit hours when you matriculate. (Ie. Like you may need 6 credits of English, but you can apply without meeting that requirement as long as you meet it by the time you start)

Seems like there may be a bit of variability among schools. UMiss for example states :
For UVA which you are interested:
  • Applicants who are not U.S. citizens or permanent residents of the U.S. are eligible to apply provided they have completed at least 90 semester hours of coursework, at the time of application, in a U.S. or Canadian college or university.
I would interpret that as exclusive of AP credits.

Having dealt with policies far too long, they are often written inconsistently across the websites (and often not reviewed by general counsel). The Priztker requirement as below actually implies AP counts as it use AMCAS methodology. Mississippi is kind of all over the map on that web page but appears to mean 90 graded credits for admission and not 90 to apply. (frankly that had to be one of the most confusing requirements page I have read) That is the kind of place I call/email and ask for clarification. Lastly, all schools rely on AMCAS for this credit requirement check as they do not see the original transcripts. So if AMCAS has the 90 credits, most schools will take it as such.

So when looking at a schools specific requirements does it ask for
90 credits to apply
90 credits for admission
90 credits to matriculate

Just to add to the, the 90 credit rule at some schools when it applies to international at some schools is bipolar or even schizophrenic. I had the experience several years ago with JHU which had the statement about all applicants having to had attended an accredited US/Canadian College for the 90 credits but the next paragraph read about international students Then the next paragraph having foreign degree holders needing 32 credits which contradicts the first paragraph.

All required courses must have a college grade; therefore, advanced placement credit cannot meet these requirements. If an applicant has advanced placement credit for any required course, he/she can fulfill the requirement by taking either that course for a grade or a higher-level course in the same department for a grade. UVA states that "Applicants should note that the University of Virginia School of Medicine considers applications only from applicants who have completed a minimum of 90 semester hours or who have graduated from an accredited college or university in the United States, Canada, or the United Kingdom."

Do these minimum hours include AP credits? Or, is it only credits earned through the college?

If they don't count AP credits, then I can't apply to UVA because I will be taking a gap year junior year and wont have 90 credits (without AP) after junior year to apply. But, if they accept AP, then I will have met their requirement. Please let me know what to do.
AP's definitely count, as long as they are somewhere on your official transcript.
