Would it be possible to somehow delete or edit the names. I didn't put MD after their names and for the title I didn't realize it had to be in title/department format. I put chair of internal medicine instead of chair/internal medicine. I feel like little things like this make you look bad, is there any way to fix this?

according to the instructions you have to make another entry for the LOR writer. Just make sure whoever is uploading the letter (i.e. your LOR writer or Dean's Office) knows which one is the correct entry

If it makes you feel any better, I had no idea what to put for that section either. I just put stuff like "internal medicine" or "psychiatry" in that box for the title/department of the letter writer. Hope it doesn't really matter.

I talked to the ERAS representatives and it doesn't matter. Don't fret over this. Just get the title (Professor vs. Assoc. Professor, etc.) and Department (Medicine vs. Surgery vs. Pathology) right.

Agreed you are really overthinking this. That said, nothing stops you from creating another slot for the same person, finalizing it, and giving them a new cover letter. But you're worried about nothing.
