Elective at Mayo Clinic

Guys,
Has anybody rotated at the Mayo Clinic? Would appreciate some advice with the applications.
How competitive is the elective with a usmle step 1 edging on the high 99s?
I'm an international medical student, and have absolutely no idea of what it takes to get to do an elective at Mayo. Would be glad for any input.

Does prior research play a significant role?

What does edging on the high edge of 99s mean? In the US we use the three digit score.
