Dear Registrar:

I hereby request that you forward my official transcript to AMCAS, the American Medical College Application Service. AMCAS accepts PDF eTranscripts from approved senders. Please visit www.aamc.org/amcasregistrars to learn how to become an approved sender. For those schools not yet approved to send PDF eTranscripts, please attach this form and mail my transcript(s) to the address below.


If schools were to send them electronically, am I to assume they'll still attach my request form?
 
AAMC can get your transcripts without the form, the form just speeds up the process and fewer mistakes are made. But I had a clearinghouse send transcripts without the form, and they were added to my file just fine.

Do you remember the name of the clearinghouse you used for your transcripts? My school uses a clearinghouse (National Student Clearinghouse) as well, but it only mentions AMCAS once as an organization to send them to and I am not sure of how to make sure they get there. Any insight?
